Private Duty Nursing, Rehabilitative Therapies, Care Management & Home Care Services
Professional and paraprofessional services are offered from licensed and/or certified offices from licensed professionals. Services include Registered Nurses, Licensed Practical/Vocational Nurses, Physical, Occupational and Speech Therapists, Medical Social Workers, Home Health Aides and Homemakers. Close monitoring may be required after a hospitalization, or to shorten or avoid hospital stays, and enable an individual to stay at home during acute illnesses. These services can also be combined with Care Management or Paraprofessional Care Services.

Care Management Services help older or disabled adults and family members determine what their specific needs are, and which services are available to meet those needs. Older persons in need of long term care will first meet with a care manager to discuss daily living needs, activities they can do on their own, as well as those that require help. The care manager can then proceed to advocate for and help clients to source the services needed to continue living in their own home or community. The care manager periodically checks with the client and family members providing support and communication

Care services may include assistance with bathing, dressing, housekeeping and laundry, meal preparation, medication reminders, shopping and errands. Services can be arranged for a visit or up-to twenty-four hour care or on a respite or temporary basis.
